A Patient Centered Medical Home is not a building rather it is a "home base" for your health care needs. It is focused on the way care is given, the people who provide the care and the place where the care is given putting patients at the core of their healthcare.
The goal of a Patient Centered Medical Home is to actively involve our patients in their health care goals, support you in achieving these goals while continuing to respect you as a unique individual and providing you with high quality care and advice. We are committed to respecting your privacy and provide timely and round the clock access.
Your role in the Patient Centered Medical Home is to be an active participant in your health care by communicating your health care needs with your physician, assuring that we receive copies of reports from any other physician that you go to, and most importantly commit to carrying out the Health Plan set forth for you by your physician.
Our Board Certified Physicians and Family Nurse Practitioner and highly trained staff are committed to providing evidenced based quality and compassionate care at the highest standards for every patient in the communities we serve regardless of age, race, color or creed.
